What happens if you go in the den and just stand there for a minute or tow? Does the arrow eventually see that you're in the cave and start to work?

It doesn't appear to, no. It stays pointing towards the exit saying "Exit cave". If I right-click on the arrow and tell it to re-check the route it very briefly points into the cave saying "Corrupt the heart" then immediately spins round to the cave exit and says "exit cave" again.

Just as an FYI, I'm having the same problem with the most up to date guides 6.0.14481. My problem however is not limited only to caves. The navigation system has been really messed up since even before 6.0. Since I started playing again and using the guides, the waypoint system is constantly telling me the wrong way to go. It will tell me to go back to the beginning of a string of waypoints and then when I reach one it wants me to turn around again. Just before legion, there was an instance where it had me in an infinite cycle of teleporting to the monk area and through several different portals and then BACK to the monk area again, and never actually leading me to where it wanted me to go. Sometimes it tells me to fly or hearth to some crazy far away location to get to a point I should just be walking to 100 yards away. I almost can't rely on the navigation at all anymore

We had a big update go out today. Pleas see if that has helped at all with your travel system problems.

It might be doing better, I've only been in one cave so far, but it seemed to be ok. However just now I started following the Talador guide while in Gorgrond so it had me heading south. As soon as I got really close to the border between the two zones, suddenly it wants me to head east to Bastion Rise, fly up to Beastwatch, and then head south again. I was like 5 or 10 seconds from the border >.>

We see some wonky things now and then still, but it seems to be working better than it did.
