In the Southern Barrens, the quest Claim the Battle Scar requires you to scout the flag 10 times and kill 1 Triumph Vanguard, according to the step display. This is backwards. The kill count does count kills properly, even if it says you're scouting the flag again when you do. It's somewhere around step 90, if that helps (I already turned it in so I can't be sure).
